Every wild Pokémon encounter has been replaced with the weakest, most poorly-statted options in the National Dex. Your starters are Slugma, Paras and Goldeen. Healing items are gone. EVs are removed. Setup moves cost 1PP. Many powerful moves have had their PP severely reduced. And at key points throughout Kanto, the game locks you into gauntlets — stretches where you cannot leave until you've defeated every trainer in sequence.

The result is a genuinely brutal resource management challenge built for Nuzlocke runs, where every decision matters and there are no easy outs.
